RESUMEN

American cutaneous leishmaniasis (ACL) is the most prevalent form of leishmaniasis, associated with an ulcerative and stigmatizing mucocutaneous pathology. This study assessed the incidence of Leishmania (Viannia) braziliensis in members of the Argentine Army who were exposed to sandfly bites in Iguazú National Park (INP), northeastern Argentina, during an outbreak of ACL in 2019, and the presence of Leishmania in rodents, opossums and phlebotomine sandflies collected in the area of exposure. Samples from military personnel, wild animals and phlebotomine sandflies were analysed. A total of 20 (40%) patients among the Army personnel and two Akodon montensis rodents (11%) were positive for the presence of Leishmania sp. genes by PCR, while Nyssomyia whitmani and Migonemyia migonei, competent vectors of Leishmania, were also found at the same site. Sequences of hsp70 DNA fragments obtained from human samples confirmed the identity of L. (V.) braziliensis. The risk to which military personnel carrying out activities in the forest are exposed is highlighted, and this risk extends to any worker and visitor who circulates without protection in the INP, coming into contact with transmission "hot spots" due to the concentration of vectors, reservoirs and/or parasites.

Migonemyia migonei (FranÒ«a, 1920) (Diptera: Psychodidae) belongs to the subfamily Phlebotominae, of epidemiological importance due to its role as a vector in leishmaniasis transmission cycles and its broad geographic distribution in South America. Few morphometric and genetic studies have demonstrated the existence of variability among geographically distant populations in Brazil. The aim of the study was to estimate the genetic distance within the morphospecies Mg. migonei through the analysis of cytochrome C oxidase subunit I (COI) sequences of specimens captured in Argentina and those available in online databases. The COI sequences from specimens collected in different localities of Argentina and sequences available in online databases were utilized. Genetic distances were analyzed and a median-joining haplotype network was constructed. Finally, phylogenetic reconstruction was performed according to Bayesian inference. The analyses led to the identification of at least two haplogroups: haplogroup I with sequences of specimens from Colombia, Brazil and Argentina, and haplogroup II with sequences of specimens from Argentina. Interestingly, specimens from Argentina whose haplotypes corresponded to both haplogroups, were collected in sympatry. The results suggest that Mg. migonei could be a species complex with at least two distinct members. This hypothesis could explain the known characteristics of adaptability and vector permissiveness of the species, as the putative cryptic species of the complex could differ in traits of epidemiological importance.

The degree of host specificity of fleas varies from highly specific (monoxenous) to opportunistic (polyxenous). Specific parasite-host associations can be observed among some flea families and tribes and mammalian orders, such as fleas of the tribe Tritopsyllini (Ctenophthalmidae, Doratopsyllinae) and opossums (Didelphimorphia). Specimens of the family Didelphidae are common hosts of fleas of the genus Adoratopsylla (Ewing, 1925). In northern Argentina, Adoratopsylla (Adoratopsylla) antiquorum antiquorum (Rothschild, 1904) and Adoratopsylla (Tritopsylla) intermedia intermedia (Wagner, 1901) were recorded preferentially parasitizing opossums. In order to study parasite/host relationships, fleas were collected from opossums captured in different environments in the Paranaense Rainforest ecoregion, northern Misiones province between 2016 and 2018. A total of 287 fleas were collected from 110 opossums. The fleas were identified as Pulicidae: Ctenocephalides felis felis (Bouché, 1835); Rhopalopsyllidae: Polygenis (Polygenis) rimatus (Jordan, 1932), Polygenis (Polygenis) roberti roberti (Rothschild, 1905); Ctenophthalmidae: A. (T.) i. intermedia, A. (A.) a. antiquorum, Adoratopsylla (Adoratopsylla) antiquorum ronnai (Guimarães, 1954). We report for the first time in Argentina a male specimen of A. (A.) a. ronnai collected on Didelphis albiventris (Lund, 1840) (Didelphidae), and male and female specimens of P. (P.) r. roberti collected on Didelphis aurita (Wied-Neuwied, 1826), and we describe the expansion of the geographic and host distribution of fleas to the Paranaense Rainforest ecoregion. Our records reinforce the specific association between Adoratopsylla and Polygenis fleas and didelphid opossums, mentioned in the literature for the Atlantic Forest ecoregion in Brazil. Furthermore, we observed that flea communities in opossums change in a landscape gradient, with invasive cat fleas and euryxenous fleas common in anthropized areas, and endemic fleas common in natural areas. Our results underline the importance for public health and veterinary medicine the analysis of flea circulation between wild and urban environments due to the risk of pathogen transmission.

In the present study, the temporal dynamics of the main vector of Leishmania braziliensis, Nyssomyia whitmani, was measured by monthly captures of phlebotominae sandflies during 5 consecutive years (from 2011 to 2016) in the Paranaense region of South America. The captures were performed in environments where the human-vector contact risk is high: domiciliary and peridomiciliary environments in a rural area endemic of tegumentary leishmaniasis. Nyssomyia whitmani was recorded as the dominant species of the phlebotominae ensemble in all domiciliary and peridomiciliary environments (House, Chicken Shed, Pigsty, and Forest Edge). Using generalized additive models, intra- and interannual fluctuations were observed, modulated by meteorological variables such as the minimum temperature and the accumulated precipitation 1 wk prior to capture. The installation of a pigsty by the farmer during the study period allowed us to observe and describe the so-called "pigsty effect" where the Ny. whitmani population was spatially redistributed, turning the pigsty as the environment that obtained the highest phlebotominae record counts, thus maintaining the farm overall abundance, supporting the idea that the environmental management of the peridomicile could have an impact on the reduction of epidemiological risk by altering the spatial distribution of the phlebotominae ensemble in the environments.

RESUMEN INTRODUCCIÓN El trastorno del espectro autista (TEA) se caracteriza por alteraciones en la comunicación y patrones restrictivos del comportamiento. El Aislamiento Social, Preventivo y Obligatorio (ASPO) en Argentina generó cambios en las dinámicas familiares de personas con TEA. El objetivo del estudio fue describir el impacto del ASPO en el acompañamiento de niños/as con TEA. MÉTODOS Se creó un instrumento específico de diseño mixto, con implementación de encuestas virtuales a familiares de personas con TEA. RESULTADOS Las 66 encuestas obtenidas revelaron ventajas, tales como la posibilidad de compartir tiempo en familia y conocer las necesidades de las/os hijas/os, el establecimiento de un ambiente con estímulos adecuados y ausencia de distractores, y la enseñanza con ritmos de aprendizaje y contenidos ajustados a las necesidades de niños/as con TEA. También se encontraron dificultades para la socialización: surgimiento de comportamientos no adaptativos (crisis, ansiedad, agresividad), barreras en la comunicación entre docentes y familias, imposibilidad de acceder a terapias y obstáculos para acceder a terapias virtuales. DISCUSIÓN El ASPO incidió en la vida cotidiana de las personas con TEA, de manera similar a lo observado en otras latitudes. A partir de lo experimentado durante el ASPO se proponen estrategias contextualizadas a la situación de pandemia actual y a potenciales escenarios similares.


ABSTRACT INTRODUCTION Autism spectrum disorder (ASD) is characterized by alterations in communication and restrictive behavior patterns. The Social, Preventive and Compulsory Isolation in Argentina changed family dynamics of people with ASD. The objective of this study was to describe the impact of isolation on accompanying children with ASD. METHODS A specific instrument of mixed design was created by conducting virtual surveys to relatives of ASD children. RESULTS The 66 surveys included in the study revealed advantages, such as the possibility of sharing family time and recognizing children's needs, environment with adequate stimuli and without distractors as well as teaching according to learning pace and content adjusted to ASD children. They also found difficulties in socialization: emergence of non-adaptive behaviors (crisis, anxiety, aggressiveness), communication barriers between teachers and families, therapy inaccessibility and obstacles to access to online therapies. DISCUSSION The isolation affected ASD people's daily life, similarly as in other countries. Based on the experiences during the isolation, strategies are proposed within the framework of the current pandemic situation and for potential similar scenarios.

Lutzomyia longipalpis and Nyssomyia whitmani were incriminated as vectors of Leishmania sp. Spatial heterogeneity together with seasonal changes in abundance constitute important elements for the understanding of the dynamics of vector populations, and there are fundamental for the development of adequate prevention and control strategies. The aim of this work was to compare the spatial and seasonal abundance of Lu. longipalpis and Ny. whitmani at a city spatial scale between two periods separated by three years. To study the spatial distribution, we compared the abundance distribution of these species at two warm times, 2011 with 2014. Respect to inter-annual seasonal abundance changes, we compared the four seasons of the year between two periods (2011-2012 vs 2014-2016). The spatial distribution for both species were found to be distributed mainly in the same areas of the city in both periods. We change for: Respect to the seasonal pattern of abundance, we observed that seasonal patterns showed changes between periods. Our study defines the 'where' and 'when' implement the actions to mitigate leishmaniasis cases.

Aedes aegypti (Linnaeus) and Aedes albopictus (Skuse) are worldwide vectors of dengue and yellow fever viruses. These species coexist in many countries and the biotic interactions between them can influence their abundances and distributions. In Argentina, Ae. aegypti is widely distributed in the north and center regions of the country, with temperate and subtropical climate, while both are sympatric only in the northeastern area of the subtropical region. Interspecific and intraspecific larval competition for food was evaluated to assess if their interaction influences on patterns of abundance and distribution. Finite rates of increase and survivorship for each species were estimated and the effects of mosquito density ratio and detritus availability were determined. The Lambda (λ´) index of population performance of both showed there is no competitive exclusion pattern. However, survival of Ae. albopictus was negatively affected by the presence of Ae. aegypti. These results suggest one possible explanation for the codominance pattern of both species display in rural regions of the southernmost distribution of Ae. albopictus in South America. They also show Ae. aegypti as a potential biotic barrier for the expansion of Ae. albopictus as was reported in regions of the United States.

A cross-sectional survey to assess the prevalence of Enterobius vermicularis infection and its associated factors among the child population of infant, preschool and school age in the urban, rural and indigenous population of Iguazú city, in subtropical Argentina was presented. Additionally, the status of enterobiasis at country level was reviewed and analysed. Enterobius vermicularis presence was assessed employing an oviscopic serial sampling technique. Statistical analysis of socio-demographic determinants was performed by generalized linear mixed models at individual, household and community levels. Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were used to gather national information about E. vermicularis prevalence spanning the decade 2010­2020. A total of 916 children from 470 families participated. Overall prevalence was 29.8%, with 25.3, 30.7 and 34.2% detected for children inhabiting urban, rural and indigenous villages, respectively. The multi-level analysis showed that the presence of E. vermicularis was mostly determined by individual (e.g. age, playing habits, previous pinworm infection) and household-level factors (e.g. family size, overcrowding conditions). Interestingly, WASH variables, such as waste disposal, analysed at community level were also important. Data were analysed to provide eco-epidemiological features of enterobiasis in a heterogeneous subtropical child population in the same territory but with different socio-sanitary realities. The importance of promoting multi-level actions against the determinants identified, to control this public health problem integratively was evidenced. The scoping review of national data updated the state of knowledge of this parasitosis, identifying risk determinants and gaps in knowledge at country level.

Phlebotominae are small insects distributed in the Americas from Canada to Argentina and Uruguay, counting with more than 500 neotropical species. Some of them have a vectorial role in the transmission of Leishmania Ross, the causative agent of leishmaniases, a group of worldwide distributed diseases with different clinical manifestations and transmission cycles. Our aim was to update the Phlebotominae fauna of Argentina and to make observations on the American Cutaneous (ACL) and Visceral Leishmaniasis (AVL) transmission scenarios, according to the distribution of proven or suspected Leishmania vector species and recent changes in land use. Primary data (entomological captures) and secondary data (review of 65 scientific publications with Phlebotominae records) were used. With 9 new records, 46 Phlebotominae species are now recorded through the area comprising 14 political jurisdictions and 6 phytogeographic provinces. Distribution maps were constructed for the 5 proven or incriminated Leishmania vector species, and the evidence supporting the vectorial incrimination of these species is discussed. Three main ACL transmission scenarios are described in the phytogeographic provinces of the Yungas, Chaco, and Paranaense, associated with deforestation processes, while the transmission scenarios of AVL are urban outbreaks and scattered cases in rural areas. We update the available knowledge on the Phlebotominae fauna present in Argentina, emphasizing its epidemiological relevance in the current context of the increasing frequency of ACL outbreaks and geographic spread of AVL.

The scarce information about breeding sites of phlebotomines limits our understanding of the epidemiology of tegumentary leishmaniasis. Identifying the breeding sites and seasons of immature stages of these vectors is essential to propose prevention and control strategies different from those targeting the adult stage. Here we identified the rural breeding environments of immature stages of Ny. whitmani, vector species of Leishmania braziliensis in the north of Misiones province, Argentina; then we determined and compared the environmental and structural characteristics of those sites. We also identified the season of greatest emergence and its relationship with adult abundance. During a first collection period, between 28 and 48 emergence traps were set continuously for 16 months in six environments of the farm peridomicile and domicile: below house, chicken shed, experimental chicken shed, forest edge, pigsty and under fruit tree. Traps were checked and rotated every 40 nights. A total of 146 newly emerged individuals were collected (93.8% of them were Ny. whitmani), totaling an effort of 23,040 emergence trap-nights. The most productive environments were chicken shed and below house, and the greatest emergence was recorded in spring and summer. During a second collection period, emergence traps and light traps for adult capture were placed in the chicken shed and below house environments of eight farms. Emergence traps were active continuously during spring, summer, and early autumn. Environmental and structural characteristics of each environment were recorded. A total of 84 newly emerged phlebotomines (92.9% Ny. whitmani; 72,144 emergence trap-nights) and 13,993 adult phlebotomines (147 light trap-nights) were recorded in the chicken shed and below house environments. A positive correlation was also observed between trap success of newly emerged phlebotomines and of adults after 120 days. A high spatial variability was observed in the emergence of Ny. whitmani, with the number of newly emerged individuals being highest in soils of chicken sheds with the highest number of chickens and closest to forest edge. Moreover, below house was found to be as important as chicken sheds as breeding sites of Ny. whitmani. Management of the number of chickens in sheds, soil moisture and pH, and the decision of where to localize the chicken sheds in relation to the houses and the forest edge, might contribute to reduce the risk of human vector exposure and transmission of Leishmania.

OBJECTIVE: Determine and characterize potential risk areas for the occurrence of cutaneous leishmaniasis (CL) in Latin America (LA). METHOD: Ecological observational study with observation units defined by municipalities with CL transmission between 2014-2018. Environmental and socioeconomic variables available for at least 85% of the municipalities were used, combined in a single database, utilizing the R software. The principal component analysis methodology was combined with a hierarchical cluster analysis to group clusters of municipalities based on their similarity. The V-test was estimated to define the positive or negative association of the variables with the clusters and separation by natural breaks was used to determine which ones contributed the most to each cluster. Information on cases was also incorporated in the analyses to attribute CL risk for each cluster. RESULTS: This study included 4,951 municipalities with CL transmission (36.5% of the total in LA) and seven clusters were defined by their association with 18 environmental and socioeconomic variables. The historical risk of CL is positively associated with the Amazonian, Andean and Savannah clusters in a decreasingly manner; and negatively associated with the Forest evergreen, Forest/crop and Forest/populated clusters. The Agricultural cluster did not reveal any association with the CL cases. CONCLUSIONS: The study made it possible to identify and characterize the CL risk by clusters of municipalities and to recognize the epidemiological distribution pattern of transmission, which provides managers with better information for intersectoral interventions to control CL.

BACKGROUND: Visceral leishmaniasis (VL) is a zoonotic protozoal vector-borne disease that is a major public health challenge. In Argentina, canine (CVL) and human visceral leishmaniasis (HVL) have recently emerged. There is a lack of standardised diagnostic tests for CVL, which hinders control of CVL and HVL. METHODOLOGY/PRINCIPAL FINDINGS: Sampling was carried out in Puerto Iguazú, Argentina, comprising 190 asymptomatic, oligosymptomatic and polysymptomatic dogs. The following diagnostics were applied: microscopy of lymph node aspirate (LNA); three immunochromatographic rapid diagnostic tests (RDTs), prototype rK28-ICT, rK39-ICT (both Coris BioConcept), commercial rK39 (InBios); ELISA for IgG, IgG1 and IgG2, against rK28, rK39 or crude lysate antigen. DNA detection and analysis, with 30 dogs, was of the ITS1 region using skin samples, and loop-mediated isothermal amplification (LAMP; Eiken Loopamp) of buffy coat, skin scrape or LNA. 15.4% of dogs were positive by LNA microscopy. The rK28 RDT had higher seropositivity rate (61%) than either a prototype rK39 RDT (31.4%) or commercial rK39 RDT (18.8%), without cross-reactivity with six other pathogens. IgG anti-rK39 ELISA antibody titres, but not IgG2, were positively correlated with number of clinical signs. LAMP with LNA had a higher positivity rate than PCR; buffy coat sampling was more sensitive than skin scrape. ITS1 confirmed Leishmania (Leishmania) infantum as the agent of CVL. Leishmania (Viannia) spp. was detected in skin samples from two dogs, compatible with Leishmania (Viannia) braziliensis. CONCLUSIONS/SIGNIFICANCE: Seroprevalence confirmed rapid increase in CVL in Puerto Iguazú. The rK28 RDT test potentially has great value for improved point-of-care diagnosis. Given cost reduction and accessibility, commercial LAMP may be applicable to buffy coat. RDT biomarkers of CVL clinical status are required to combat spread of CVL and HVL. The presence of Viannia, perhaps as an agent of human mucocutaneous leishmaniasis (MCL), highlights the need for vigilance and surveillance.

Estudio observacional ecológico con unidades de observación definidas por municipios con transmisión de LC entre 2014-2018. Se utilizaron variables medioambientales y socioeconómicas disponibles para al menos 85% de los municipios, combinados en una sola base de datos, a través del software R. Se combinó la metodología de análisis de componentes principales con un análisis de conglomerados jerárquicos para la formación de conglomerados de municipios en función de su similitud. OBJECTIVE: Determine and characterize areas at potential risk for the occurrence of cutaneous leishmaniasis (CL) in Latin America. METHOD: Ecological observational study with observation units defined by municipalities with CL transmission during 2014-2018. Environmental and socioeconomic variables available for at least 85% of municipalities were combined in a single database, using R software. Principal component analysis was combined with hierarchical cluster analysis for the formation of clusters of municipalities according to their similarity. The V-test was used to define positive or negative association of variables with clusters and separation by natural divisions to determine which contributed more to each cluster. Cases were included to attribute CL risk for each cluster. RESULTS: The study included 4 951 municipalities with CL transmission (36.5% of municipalities in Latin America); seven clusters were defined by their association with 18 environmental and socioeconomic variables. Historical risk of CL is associated positively and in descending order with the Amazonian, Andean, and Savanna clusters; and negatively with the Forest/perennial, Forest/cultivated, and Forest/populated clusters. The Agricultural cluster showed no association with CL cases. CONCLUSIONS: The study made it possible to identify and characterize CL risk by clusters of municipalities and to understand the characteristic epidemiological distribution patterns of transmission, providing program managers with better information for intersectoral interventions to control CL.

Background: Cutaneous leishmaniasis is an ancient endemic disease in Iran and continues to be a growing health threat to community development and the environment. This paper explains how to use the facilities of health centers for developing a laboratory network on vectors and reservoir hosts of cutaneous leishmaniasis in Iran. Methods: A literature search was performed of the relevant multiple databases to include studies on vectors and reservoirs of cutaneous leishmaniasis in Iran. A team of experienced experts was performed. After holding several meetings, field visits and organizing workshops, the activities of laboratories were determined at three levels. Results: Entomological studies showed that 5 species of the genus Phlebotomus and 10 species of the genus Sergentomyia are active in the south, 4 species of the genus Phlebotomus and one species of the genus Sergentomyia in the central part and 5 species of the genus Phlebotomus and 2 species of the genus Sergentomyia in the north east. Reservoir hosts were identified in the study areas. The tasks of laboratories were regulated at different levels. Conclusion: The Iranian Ministry of Health and Medical Education should prioritize the employment of capable persons in the field of Medical Entomology and Vector Control. The survival of this laboratory network depends on hiring and employing interested and persistent people. The universities of Medical Sciences that have the facilities to set up this network will be a very effective partner in the control of the disease in high risk areas. The results can be used in neighboring countries.

BACKGROUND: In South America, cutaneous leishmaniasis (CL) and visceral leishmaniasis (VL) are emerging diseases, expanding in the border area of Argentina, Brazil and Paraguay. Outbreaks of CL were reported since the 1990s, with Nyssomyia whitmani as the main vector in this region. Regarding VL, urban reports started in 2010 with Lutzomyia longipalpis as the main vector. The aim of this study was to evaluate environmental determinants related to the main vectors of leishmaniasis, to contribute to the prevention and control response to the emergence of VL and CL in the Argentina-Brazil-Paraguay border region. METHODS: The cross-sectional survey includes two cities and two close rural areas in the Argentinean Northeast Region, between November 2014 and January 2015, with a total of 95 sampling sites. REDILA-BL traps were set for three consecutive nights, and a total of 68 meso- and microscale environmental and landscape characteristics were surveyed. The association between vector abundance with different variables was evaluated using a generalized linear model with zero-inflated negative binomial distribution. We analyzed females for detection of Leishmania DNA. RESULTS: The analysis for Lu. longipalpis indicates an excess of absences when the mean NDWI around the sites were higher. The abundance of Lu. longipalpis at mesoscale level was higher when more urban services were present, and when blood sources such as chickens or dogs at the microscale level were present. For Ny. whitmani, no variable was found to be associated with the absences, while its abundance increased in association with the following variables: percentage of tree cover, presence of garbage collection service, hosted people and, at microscale, the presence of poultry. Leshmania infantum DNA was detected in 2/49 (4%) Lu. longipalpis. CONCLUSIONS: The abundance of both species is influenced by variables at different scales, their influence probably has a hierarchy and they are acting on different aspects of the biology of these vectors. The urban spatial segregation of Lu. longipalpis and the peri-urban and rural segregation of N. whitmani increase the risk of VL and CL. The selection of the better variables for each scale will allow the design of appropriate control strategies depending on species.

BACKGROUND: The Nyssomyia genus and Lutzomyia subgenus include medical important species that are Latin American leishmaniases vectors. Little is known about the phylogenetic relationships of closely-related species in each of these taxonomic groups that are morphologically indistinguishable or differentiated by very subtle details. OBJECTIVES: We inferred the phylogenetic relationships of closely-related species within both the Nyssomyia genus and the Lutzomyia subgenus using a cytochrome c oxidase subunit I (COI) fragment. METHODS: The sampling was carried out from 11 Argentinean localities. For genetic analyses, we used GenBank sequences in addition to our sequences from Argentina. Kimura 2-parameter (K2P) genetic distance and nucleotide divergence (Da) was calculated between closely-related species of Nyssomyia genus, Lutzomyia subgenus and between clades of Lutzomyia longipalpis complex. FINDINGS: The K2P and Da values within species of Nyssomyia genus and Lutzomyia subgenus were lower than the divergence detected between clades of Lu. longipalpis complex. The haplotype network analyses within Lutzomyia subgenus showed shared haplotypes between species, contrary to Nyssomyia genus with none haplotype shared. Bayesian inference within Nyssomyia genus presented structuring by species. MAIN CONCLUSIONS: This study evidences the phylogenetic proximity among closely-related species within Nyssomyia genus and Lutzomyia subgenus. The COI sequences of Nyssomyia neivai derived from the present study are the first available in GenBank.
